Where do the signed forms go?

Into your practice's Google Drive: one PDF per patient, plus a spreadsheet that logs every submission. Not using Google Drive yet? We keep them in a secure folder shared with your practice email.

Tablet at the front desk, or the patient's own phone?

Either. Tell us when you upload, because it changes how we build it. On a shared tablet, each patient's answers are cleared before the next patient starts.

How fast is it, really?

Your first version arrives within 24 hours of your confirmation. We build two forms a day at that speed; on a busier day the next ones get 48 or 72 hours, and you see the exact time before you confirm.

What if we barely use it in the first month?

If the form is used fewer than 10 times during your free month, we pause it and charge nothing. You can reactivate it within 30 days. Your patient records are never deleted.

Do we need to install anything?

No software. Your form opens from its own address and can be added to the home screen of a tablet or phone, like an app.
