One form completed and submitted by a real patient. Test submissions, including the ones you run while reviewing the preview, never count.

We have several forms. How does that work?

Each form has its own price, its own free month and its own count. You only pay for the forms your patients actually use.

What happens if our form gets paused?

You get an email with a button to reactivate it at the same monthly price. You have 30 days. Your patient records are never touched.

Is there a setup fee?

No. Building your form from your paper is included in the price.

What does Yanforms Sign cost?

Yanforms Sign is our e-signature tool for your practice's own documents, such as attendance sheets, policies and agreements. Free and unlimited for practices with an active Yanforms form. Otherwise, 10 documents a month free, then $9 a month.

Q: Is every form $75?

No. $75 is the starting price. Most forms stay at $75 a month; long or complex forms, such as multi-document intake packets, are up to $120. We confirm your form's price with your preview, before you approve it.

Most software asks you to pay first and hope it gets used. We flipped it. Your first month is free, and from month two you pay only if your patients actually used the form.

Billing questions, answered

No. We build your form first, with no card. We ask for one before your first billed month, and nothing is charged unless your form was used at least 10 times during your free month.

Yes. Pay 3 months ahead and save 10%, 6 months and save 15%, a full year and save 20%. On a $75 form that is $67.50, $63.75 or $60 a month. You pick your plan when you send us your form and can change it any time before your first billed month. The free month and the 10-use rule do not change: if patients used the form fewer than 10 times, nothing is charged, whatever plan you picked. A prepaid period is paid in one go and is not refunded if you stop early; your form stays live until the period ends.
