Yanforms

OMHC intake forms, complete before the first session.

An outpatient mental health center opens each record with a packet: consents to treatment, a rights acknowledgment, history and allergies, releases of information, insurance. Yanforms turns the packet you already use into one form your clients fill on a phone or a tablet, with each answer typed once and one signed PDF per client in your Google Drive.

One packet, one signature.

In short

  • Your own intake packet, page for page, in your wording, as one form.
  • Name, date of birth and signature entered once, then carried to every page that asks for them.
  • Built with what record reviews look for: a signed rights acknowledgment, allergies that cannot be skipped, releases signed or declined.
  • Insurance card and ID photographed in the form, with the member ID as a required field.

What Maryland calls an OMHC

Under COMAR 10.63.03.05, an outpatient mental health center provides regularly scheduled outpatient mental health treatment in a community-based setting: at a minimum individual, group and family therapy and medication management, with a psychiatrist as medical director and multidisciplinary clinical staff.

For its Medicaid clients, its claims go through Maryland’s public behavioral health system. In Maryland, Carelon Behavioral Health has been the administrative services organization of the public behavioral health system since January 1, 2025. The Maryland Department of Health lists authorizations, utilization, claims processing, provider management, auditing and provider compliance among its responsibilities.

What a record review looks for at intake

Carelon’s training on treatment record reviews (revised February 2023) lists, among other items: a member rights and responsibilities statement whose receipt the member acknowledges by signing; medications, allergies and medical history that are easy to find in the record, where no known allergies is a valid answer; and a signed and dated release of information, or documentation that the member refused to sign.

Its other items, such as the treatment plan and progress notes, belong to your clinical record, which Yanforms does not write.

How we build your intake packet

We rebuild the packet page for page and keep your legal text word for word. The client types each answer once, it is carried to every page that asks for it, and the client signs once. The date of signature is recorded automatically.

We build it with the items above: the rights or handbook acknowledgment signed in the same sitting, an allergies question that cannot be skipped and where None is an answer, and a release of information the client signs or declines, with the refusal written into the signed PDF. Consents to treatment and telehealth consent can be part of the same packet.

Insurance and member ID at the door

Clients photograph their insurance card and ID right in the form, inside a card-shaped frame. The member ID can be a required field: the form cannot be sent with it blank.

Each completed form becomes one signed PDF plus one row in a Google Sheet, filed in your practice’s own Google Drive. Not using Google Drive yet? We keep them in a secure folder shared with your practice email. Yanforms’ servers receive a count of uses, never what a patient wrote.

Questions, answered

Can our whole intake packet become one form?

Yes. Long packets become one form and one signed PDF per client. Long or complex forms are priced up to $120 a month, and the price is confirmed with the preview, before you approve it.

Can a client decline to sign a release of information?

Yes, if your form allows it. We build the release so the client signs it or declines it, and the refusal is written into the signed PDF. Carelon’s record review training lists a documented refusal as one way to meet that standard.

Can clients fill the intake before their first appointment?

Yes. Your form has its own address: send it before the appointment and clients arrive already done, or hand them a tablet in your office. On a shared front-desk tablet, each patient’s answers are wiped before the next patient starts. No one sees the person before them.

Is it HIPAA-ready?

Yanforms signs a Business Associate Agreement with every practice, free of charge, and publishes the full text at yanforms.com/baa. Your form is not used with patients before it is signed. Completed forms are handled inside Google Workspace, which is covered by Google’s own BAA.

What does an OMHC intake form cost?

The first month is free, from the day the form goes live. From month two a form costs $75 a month, up to $120 for long or complex forms, and the exact price is confirmed with the preview, before you approve it. Nothing is charged unless patients used the form at least 10 times during the free month. No setup fee, no card to start.
