What is Yanforms?
Yanforms turns a clinic’s paper intake forms into HIPAA-ready web forms. You send a blank copy of your form, we rebuild it as an app your patients fill out on a tablet or their own phone, and every signed PDF lands in your practice’s Google Drive. Built for clinics in the United States and Canada.
Who is it for?
Medical, dental, behavioral health and physical therapy practices in the United States and Canada, from single-provider offices to multi-site groups. Patients fill the form on a front-desk tablet or on their own phone.
What does it cost?
The first month is free, from the day the form goes live. From month two a form costs $75 per month, up to $120 for long or complex forms, and the exact price is confirmed with the preview before the practice approves it. Nothing is charged unless patients used the form at least 10 times during the free month. There is no setup fee and no card is needed to start.
Is there a discount for paying in advance?
Yes. 3 months in advance saves 10%, 6 months in advance saves 15%, 1 year in advance saves 20%. A practice picks its plan when it sends its form, and can change it any time before the first billed month.
Where do completed forms go?
Into the practice’s own Google Drive: one signed PDF per patient, plus one row per submission in a Google Sheet. Yanforms’ own servers never receive what a patient writes; they receive a count of uses and nothing else.
Is it HIPAA-ready?
Yes. Yanforms signs a Business Associate Agreement with every practice, free of charge, and publishes the full text at yanforms.com/baa. Completed forms are handled inside Google Workspace, which is covered by Google’s own BAA.
What about Canadian practices?
Yanforms acts as the practice’s service provider under PIPEDA and Quebec’s Law 25. Completed forms are stored in Google Workspace, which may keep them outside Canada, and the practice is given what it needs to tell its patients and, in Quebec, to carry out its privacy impact assessment.
How fast is a form built?
The first version arrives within 24 hours of confirmation. On a busy day the next forms get 48 or 72 hours, and the practice sees the exact delivery time before confirming. If that time is missed, a second free month is added to the form.
How is it different from a form builder?
A form builder hands you a blank canvas and a list of field types. Yanforms hands you your own form, already rebuilt page for page, in your wording, with the logic a clinic needs: questions that only appear when they apply, name and date of birth carried to every page, a signature captured once, and a shared tablet wiped between patients. Changes are unlimited until the practice approves it.
Does Yanforms do e-signatures?
Yes, with Yanforms Sign, for the documents a practice’s team signs: meeting minutes, supervision notes, attendance sheets, agreements, policies. People sign on their own phone, with no app and no account. Free and unlimited for practices with an active Yanforms form. Otherwise, 10 documents a month free, then $9 a month.
Who is behind Yanforms?
Yanforms is built and run by Yanis Tiam, who signs the Business Associate Agreement with each practice and builds each form himself. That is why forms are delivered two a day, and why the delivery time is announced before a practice confirms.
