Yanforms

Intake and consent forms for behavioral health.

OMHCs, PRPs, substance use programs and therapy practices ask a lot on paper: history, screening, consent to treatment, telehealth consent, releases of information, a rights acknowledgment. Yanforms turns the forms you already use into one form your clients fill on a phone or a tablet, and every signed copy lands in your practice’s Google Drive.

Your questions, in your wording.

In short

  • Used by 6 behavioral health practices in Maryland and Washington, DC.
  • Your intake packet and consent forms, rebuilt page for page, in your wording.
  • Questions that only appear when they apply; allergies and member ID can be required.
  • HIPAA-ready, with a free Business Associate Agreement.

The forms we build for behavioral health

Consent to treatment for each of your programs, such as OMHC and PRP, behavioral health screening, telehealth consent, authorization to exchange information, rights and handbook acknowledgment, financial policy, and your new client intake. For PRP visits in the community, a visit record with the place, a photo and both signatures. Long packets become one form, and one signed PDF per client.

Questions that only appear when they apply

A form builder hands you a blank canvas and a list of field types. Yanforms hands you your own form, already rebuilt page for page, in your wording, with the logic a clinic needs: questions that only appear when they apply, name and date of birth carried to every page, a signature captured once, and a shared tablet wiped between patients. Changes are unlimited until you approve it.

For Medicaid programs: what reviews and claims ask for

We build your forms with the items a treatment record review looks for, such as a signed rights acknowledgment, allergies that cannot be skipped and releases of information signed or declined, and with the fields your claims depend on, such as the member ID or the referring practitioner’s NPI, as required fields. Our guides on OMHC intake forms, PRP documentation in Maryland and 42 CFR Part 2 consent go into the details, with their sources.

On the client’s phone or in your office

Tell us when you upload whether clients fill the form on their own phone or on a tablet in your office, because it changes how we build it.

On a shared front-desk tablet, each patient’s answers are wiped before the next patient starts. No one sees the person before them.

Records in your Drive, a BAA for free

Each completed form becomes one signed PDF plus one row in a Google Sheet, filed in your practice’s own Google Drive. Not using Google Drive yet? We keep them in a secure folder shared with your practice email. Yanforms’ servers receive a count of uses, never what a patient wrote.

Yanforms signs a Business Associate Agreement with every practice, free of charge, and publishes the full text at yanforms.com/baa. Your form is not used with patients before it is signed. Completed forms are handled inside Google Workspace, which is covered by Google’s own BAA.

Questions, answered

Can you rebuild our consent to treatment and telehealth consent forms?

Yes. Send a blank copy of each form you use. Each one is rebuilt in your wording, and you approve it before any client uses it.

Do you work with Medicaid programs such as OMHCs and PRPs?

Yes. Yanforms is used by 6 behavioral health practices in Maryland and Washington, DC. We build intake packets, consents, releases of information and PRP visit records, with the required fields your record reviews and claims depend on.

Can a long intake packet be one form?

Yes. Long or complex forms, such as multi-document intake packets, are priced up to $120 a month, and the price is confirmed with the preview, before you approve it.

Is it HIPAA-ready?

Yanforms signs a Business Associate Agreement with every practice, free of charge, and publishes the full text at yanforms.com/baa. Your form is not used with patients before it is signed. Completed forms are handled inside Google Workspace, which is covered by Google’s own BAA.

What about practices in Canada?

Yanforms acts as the practice’s service provider under PIPEDA and Quebec’s Law 25. Completed forms are stored in Google Workspace, which may keep them outside Canada, and the practice is given what it needs to tell its patients and, in Quebec, to carry out its privacy impact assessment.
