Yanforms

A sign-in sheet nobody else can read.

On a paper sign-in sheet, each patient can read the names written above their own. Rebuilt as a form on the front-desk tablet, each patient signs in alone, and the tablet forgets them before the next one.

One patient, one screen, one file.

In short

  • Your own sign-in sheet, rebuilt as a form on the front-desk tablet.
  • Each patient’s answers wiped before the next patient starts.
  • Every visit filed as its own signed PDF, plus a row in a Google Sheet.
  • Priced like any other form, with a free first month.

The trouble with a shared sheet

A clipboard at the front desk shows every name and time written before. A sign-in form on a tablet shows each patient only their own screen.

On a shared front-desk tablet, each patient’s answers are wiped before the next patient starts. No one sees the person before them.

One file per visit

Every sign-in is a submission: one signed PDF and one row in your Google Sheet, in your practice’s Google Drive. A patient who comes every week has one file per visit, which is exactly what a visit log should be.

Your sheet, rebuilt

You send a blank copy of the form you already use: a PDF, or photos of the paper pages (JPEG, PNG, or HEIC from an iPhone). No card is needed to send it, and nothing is charged before your form is live.

We rebuild it with your own fields and wording, and you approve it before any patient uses it.

Staff sign-in is a different job

A staff training or a team meeting needs an attendance sheet signed by the team, not a patient form. That is what Yanforms Sign is for.

Questions, answered

Can patients see who signed in before them?

No. On a shared front-desk tablet, each patient’s answers are wiped before the next patient starts. No one sees the person before them.

Where do the sign-ins go?

Each completed form becomes one signed PDF plus one row in a Google Sheet, filed in your practice’s own Google Drive. Not using Google Drive yet? We keep them in a secure folder shared with your practice email. Yanforms’ servers receive a count of uses, never what a patient wrote.

Can we keep the fields of our current sheet?

Yes. We rebuild your own sheet, page for page, in your wording, and changes are unlimited until you approve it.

What does a digital sign-in sheet cost?

The first month is free, from the day the form goes live. From month two a form costs $75 a month, up to $120 for long or complex forms, and the exact price is confirmed with the preview, before you approve it. Nothing is charged unless patients used the form at least 10 times during the free month. No setup fee, no card to start.
